Our interdisciplinary hospice team creates a unique care plan that will accommodate each patient’s needs and concerns. Hospice is the only Medicare benefit that includes palliative and hospice-diagnosis related medications, medical equipment and 24/7 day a week support for loved ones and family. Hospice care is normally delivered at home; however Robinson Creek is able to provide hospice care any place a patient calls home.
For the first time, one out of three families in our country are now dealing with a life-limiting illness of a loved one. Most do not know where to turn to for answers and support. Robinson Creek Hospice can help in these difficult times.
Robinson Creek is the originator of the “Bridge Program”. When patients transfer from our home health to our hospice the same knowledgeable office and field staff provide a transparent transition and continuity of care.
Professional care and concise communication between patient, family members and physician are hallmarks of the Robinson Creek Hospice mission.
